Create an account at https://medmj.ehawaii.gov/medmj/welcome and then start an application for your 329 medical cannabis card. When you come across the first part where it says Physician/Providers name type in Cherie Thomas. (License MD-9100) Once you have completed the online application and have paid the $38.50 state fee for your one year 329 registration card please, call or text 808-212-8111 to schedule appointment to complete the approval process and make payment of $150 for the consult fee. or Call or text to make mobile appointment and you will just need to sign two papers, have a current form of ID, and the consult fee $150 and state fee $38.50

Delete previous physicians name and type Cherie Thomas
Pay the $38.50 state fee and Submit application

ELIGIBLE DEBILITATING MEDICAL CONDITIONS
As defined in section 121 of HRS-329 , a qualifying debilitating medical condition includes:
- Amyotrophic Lateral Sclerosis (added effective Dec. 19, 2017 per petition process)
- Cancer
- Glaucoma
- Lupus (added effective June 29, 2017 as per Act 041, SHL 2017)
- Epilepsy (added effective June 29, 2017 as per Act 041, SHL 2017)
- Multiple Sclerosis (added effective June 29, 2017 as per Act 041, SHL 2017)
- Rheumatoid Arthritis (added effective June 29, 2017 as per Act 041, SHL 2017)
- Positive status for human immunodeficiency virus
- Acquired immune deficiency syndrome
- Post-traumatic stress disorder (added effective July 1, 2015 as per Act 241, SLH 2015)
- “The treatment of these conditions”, or “A chronic or debilitating disease or medical condition or its treatment that produces one or more of the following:
- Cachexia or wasting syndrome
- Severe pain
- Severe nausea
- Seizures, including those characteristic of epilepsy
- Severe and persistent muscle spasms, including those characteristic of multiple sclerosis or Crohn’s disease
- Changes to the list:
- Act 241, SHL 2015 – Hawaii’s Dispensary Legislation, added post-traumatic stress disorder (PTSD) as a new condition effective July 1, 2015
- Act 041, SHL 2017 added lupus, epilepsy, multiple sclerosis, and rheumatoid arthritis effective June 29, 2017
- Amyotrophic Lateral Sclerosis (ALS) was added in 2017 through the petition process.
